Mail Order Pharmacy Policy

DDA physicians will not fax new prescriptions to mail order pharmacies. For new prescriptions we will continue to serve our patients’ needs by mailing written prescriptions to their home address or having them available for pick-up. This will facilitate more efficient and timely acquisition of medications.
We will continue to process standard pharmacy
refills by fax and phone. This includes mail order and local pharmacies. Please contact your pharmacy first for any medication refills needed and have a request form faxed to us. Our pharmacy fax line for refills is 972-758-6003.
